In this role, you will be responsible for conducting comprehensive assessments of patients during the admissions process, ensuring that all medical needs are identified and addressed.

You will work closely with patients and their families to provide education and support, while also collaborating with interdisciplinary teams to develop individualized care plans.

Schedule: 12pm - 10pm ( Alternating Weekends)

Responsibilities

• Conduct thorough patient assessments upon admission, including medical history and current health status.
• Provide education to patients and families upon arrival.
• In-put patient admission orders into computer system.
• Take inventory of patient belongings upon arrival
• Review all admission documentation needed to admit resident.
• Ensure compliance with all healthcare regulations and standards of practice in senior care and geriatrics.
• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.

*Pay is negotiable pending experience.
