Registered Nurse (RN) PRN-ACT Program Dekalb/Fulton (Healthcare)
Assertive Community Treatment (ACT) team provides intensive intervention, treatment, rehabilitation, and support services to individuals with severe, persistent, and cute mental illnesses or addictive diseases. Participating in client-centered comprehensive assessments of psychiatric history, mental status, diagnosis, and physical health, education and employment and social development. Participating in the development of treatment and discharge plans based on observation of individual strengths, needs, abilities, and preferences. Provides effective follow-up and referral services that facilitate individual linkages, communication, and continuity of services. Facilitates individual-family interactions, keeping supervisor and other treatment team members informed of significant developments. Complete comprehensive nursing assessments and provide links to medical care.
Under general supervision, learns and assesses client health problems and needs. Develops and implements nursing care plans and maintains medical records. Administers nursing care to ill, injured, convalescent, or disabled clients. May advise clients on health maintenance and disease prevention or provide case management.
Duties & Responsibilities:
Administers prescribed medications, and notes times and amounts on clients' charts. Consults and coordinates with health care team members to assess, plan, implement and evaluate client care plans. Maintains accurate, detailed reports and client records. Modifies client treatment plans as indicated by clients' responses and conditions. Monitors all aspects of client care, including diet and physical activity. Monitors, records and reports symptoms and changes in clients' conditions. Observes nurses and visits clients to ensure that proper nursing care is provided. Provides education to clients and family. Provides health care, first aid, immunizations and assistance in convalescence and rehabilitation.Receives additional training, as required, to gain full proficiency and experience in all areas.Other duties as assigned.
Minimum Qualifications:
Bachelor's degree in nursing from an accredited college or university OR associate degree in nursing from an accredited college or university AND current Georgia license as a Registered Professional Nurse AND any related professional work experience.
Preferred Qualifications:
Experience working with the Severe and Persistently Mentally ill population, ACT teams, or individual identified as being frequent users of ERs, Psychiatric Hospitals, Crisis Stabilization Units, and the criminal justice system.Nursing experience.Community experience working with individuals with SPMI.Experience working in a high stress, fast-paced environment with a very emotionally and physically changing population.Experience working in multi-disciplinary teams.Ability to multi-task and manage time.
