IBH Case Manager (Main) (R&D)
Accepts newly assigned cases for which the initial Intake has been completed by a SCHC Behavioral Health Consultant or Drug and Alcohol Counselor. Completes case management assessment on all patients following an assigned protocol
Engages the patient in the development of a service plan tailored to meet the patient's unique needs, circumstances, and preferences as determined through assessment. Educates the patient to make informed choices
Works actively and collaboratively with patients' assigned medical treatment teams to help patients achieve their overall treatment plan goals
Provides ongoing supportive and/or case management functions to patients in accordance with identified problems, needs and strategies. Arranges, coordinates, and monitors services from qualified resources that are prompt, adequate, and appropriate
Links into a comprehensive network of community resources, and establishes effective working relationships with internal and external resources to facilitate patient needs being met
Accompanies patients to facilitate access to services or supports, such as applications, appointments, interviews, meetings, etc. in accordance with established SCHC Safety Guidelines. Supports patient's ability to communicate his/her needs effectively.
Documents all service contacts on a timely basis including face-to-face interviews, collateral and networking contacts, correspondence and maintains the case records in accordance with agency and regulatory standards and requirements
Document progress, advancement, and completion of goals in a timely and professional manner in the electronic health record according to established standards
Participates in interdisciplinary team meetings, and other clinic meetings as assigned
Actively participates in assigned educational activities, which further professional growth and development
Meets regularly with the immediate supervisor as a means of enhancing professional growth, reviewing and processing the provision of case management services, and dealing with appropriate administrative issues
Participates in planning and evaluation of case management program
Other duties as assigned
KNOWLEDGE, SKILLS, AND ABILITIES
Knowledge of adult learning theories and group dynamics
Knowledge of various community resources in Shasta County area
Ability to work autonomously and seek out consultation from peers and others when needed
Participates in planning and evaluation of case management program
Excellent verbal and written communication skills
Typing 35 WPM or higher
EDUCATION & EXPERIENCE
Two-Year Associate Degree
Experience with case management services or patient care in a medical environment
Strong Computer or keyboarding skills.
PREFERRED
Experience working with common health care programs
Experience working with patient registration procedures
Basic knowledge of medical or dental terminology
