• Oversees the planning, developing, and direct unit using independent judgement and discretion. Sets unit standards, develops objectives and constantly strives to improve procedures. Reviews policies, procedures, and manuals on a regular basis and recommends changes. Assists in standardizing methods by which work will be accomplished and quality will be assessed.
• Assumes responsibility for selecting, training, directing, and motivating competent department personnel. Leads team and provides proactive support, removing obstacles and providing solutions as needed. Evaluates employee performance, and provides input counselling and disciplinary actions, termination, transfers, etc. Participates in determining staffing needs of the unit. Ensures that enough personnel are available for each shift to ensure that quality care is maintained.
• Oversees development of a written plan of care (preliminary and comprehensive) for each resident that identifies problems/needs of resident, indicates care to be given, goals to be accomplished and which professional service is responsible for each element of care. Reviews resident service plan for appropriate resident goals, problems, approaches, and revisions based on nursing needs. Ensures that all personnel involved in providing care to residents are aware of resident's care plan and refer to it prior to administering daily care to resident.
• Ensures that all staff members on each shift comply with written procedures for administration, storage, and control of medications and supplies. Ensures proper management and administration of all therapies and diagnostics. Monitors medication pass, treatment schedule and corresponding documentation to ensure compliance with applicable regulations and policies.
• Completes rounds to assess residents and to determine if needs are being met in accordance with resident's request and ensure nursing service personnel are performing their work assignments in accordance with acceptable nursing standards.
• Implements, coordinates and supervises compliance with Resident Care Management Systems on unit including interdisciplinary team process.
• Reviews unit's charge of condition report daily and supervises/coordinates nursing services and care plan interventions.
• Consults with resident, his/her family, and resident's physician in planning of resident's care, treatment, rehabilitation, etc., as necessary.
• Assists in arranging emergency respiratory coverage when the need arises.
• Demonstrates knowledge of facility and departmental quality improvement activities and/or participates in quality improvement meetings. Prepares monthly Quality Control reports.
Follows up on all Quality Control in-services and plans of correction. On an ongoing basis, evaluates results of quality assurance program. Recommends any equipment and supplies needed for department as well as any departmental improvements. Confers with department personnel to solicit input on improving service and addressing problem areas.
• Reviews resident medical records as directed to ensure compliance with state and federal regulations and company policies and procedures. Responsible for record maintenance of all Quality Control and scheduling within unit.
• Reviews direct care staffs' notes as needed to ensure that they are informative and descriptive of care being provided, that they reflect resident's response to care, and that such care is provided in accordance with resident's wishes.
• Ensures corrective action has been completed on medical record audits and consultant reports.
• Participates in selection of residents for admission to facility and participates in admissions, transfers and discharges of residents.
• Periodically reviews resident's written discharge plan and participates in updating it as required.
• Assists in developing methods for coordinating nursing services with other resident services to ensure continuity of residents' total regimen of care.
• Performs administrative duties such as completing medical forms, evaluations, studies, charting, etc., as necessary. Makes written or oral reports and recommendations to DON or Administrator concerning operation of unit and in response to concerns or complaints.
• Reports the following in accordance with established facility procedures and regulatory standards: accidents and incidents; resident grievances, complaints, allegations of resident abuse and/or misappropriation of resident property; occupational exposure to blood, bodily fluids, infectious materials, and hazardous chemicals; concerns and suspected incidences of non-compliance including but not limited to hazardous conditions and equipment.
• Ensures adherence to state and federal regulations and company policies and procedures; HIPAA confidentiality standards of resident and facility information; Infection Control, Universal Precautions, OSHA and safety standards; established safety precautions and regulations in the performance of tasks and the use of equipment and supplies.
• May provide direct care support on floor and serve as Nurse Supervisor if necessary.
• Participates in and/or leads on-the-job training, in-service education and orientation programs; attends OSHA and CDC in-service training programs for hazardous communications, TB management, and blood-borne pathogens standard; serves on various committees as required by existing regulations or as appointed by Administrator.
• Works with facility consultants as necessary and implements recommended changes as required. Ensures outside services are properly supervised and completed in accordance with contract/work orders.
• Keeps abreast of current federal and state regulations, as well as professional standards, and makes recommendations on changes in policies and procedures to Supervisor or Administrator. May participate in workshops and seminars to stay current with changes in field, as well as to maintain professional status.
• Maintains reference material necessary for complying with current standards and regulations, and that will provide assistance in maintaining quality service.
• Interprets department policies and procedures to personnel, residents, consultants, visitors, family members, etc., as necessary.
• Is accessible after hours for emergency purposes.
